Child and Adolescent / Adult Mental Health Services:
Child & Adolescent Mental Health services are available to promote the mental health of children and adolescents (0-18 years) and their families through the provision of direct clinical services, consultation services and professional and public education on the subject of mental health.

For adults the main focus is client contact through the provision of psychotherapy.

Comprehensive assessments of referral problems that are impeding psychosocial functioning are provided. If the problems are more appropriate to other services, referral to the service is made.

Community Mental Health Services are available at Rock Lake Hospital with visits once per week. Daphne Andrew Peers is the C.M.H.W. for our area and may be contacted at 873-2483.

Chiropractor
Dr. Oleh Zmiyiwsky provides chiropractic care Thursday afternoon and Friday mornings. With 23 years experience in practise experience, Dr. Zmiyiwsky will capably care for your needs. For appointments please call: 204-284-2024.

Community Bath Program:
A community Bath Program is available to those in need of bathing services. The program is available from both the Rock Lake Hospital and Rock Lake Personal Care Home.

Continuing Care (Home Care)
Back to top Home Care in Central Region is a comprehensive community-based program that provides essential in-home care support to individuals, regardless of age, who require health services or assistance with activities of daily living. This program augments the resources of family and community, using a model which emphasizes promotion of care in the home, facilitation of hospital discharge and provision of alternatives to entry into the long term care system for as long as is safely possible.

Services include: Assessment of eligibility, care planning, case management, nursing services, therapy services, health teaching, personal care, meal preparation, respite/family relief, cleaning and laundry, access to Adult Day Care, assessment for the facilitation of personal care home placement and medical equipment and supplies to support the care plan.

Home Care Offices are located at Rock Lake Hospital.

Rock Lake Emergency Medical Services:
A dedicated, Accredited Level 2 Paramedic Ambulance Service providing advance life support to the community 24 hours a day, seven days a week. The service is operated out of Rock Lake Hospital. Program Coordinator is Jeff Nedohin, Rock Lake Hospital: 873-2132 (extension 6)

Equipment Rental:
Rock Lake Hospital has a variety of Health Care Equipment available for rent. Some equipment would include wheelchairs, walkers, and crutches. Contact Edith Platt, Administrative Assistant 873 -2132 for a complete list.

Services for Seniors in Group Living
Back to top The SSGL program is designed to assist the tenants in Prairie View Lodge Independent Living Suites in accomplishing their life management skills. Services provided by the Tenant Wellness Coordinator 3 days per week, will be made available in accordance with individual assessed needs. The program is meant to enhance the quality of life for the tenants and is not intended to duplicate those services already provided such at Home Care.

The Tenant Wellness Coordinator will work with the tenants, determining their needs for support in their Activities of Daily Living, enabling them to remain independent within the community. These activities may include but are not limited to:
• Placing grocery orders
• Navigating the phone system to assist in making appointments
• Arranging transportation
• Assist in making arrangement for getting to and/or from appointments
• Involving tenants in planning activities to facilitate their participation in decision making processes and assessing individual needs
• Promoting group activities that facilitate group interaction between clients and /or individual attention as required
• Plan social activities that are relevant to the seasons of the year, to current events or to cultural and religious holidays


Rock Lake Handi-Van services:
Rock Lake Handi-Van service is managed and operated out of the Town of Pilot Mound. The service is available to all residents within the R.M. of Louise. Community residents are fortunate to have reliable, caring drivers who provide great assistance as needed. The fee for this service is $5.00 one way within the R.M. of Louise with an added .50 cents per km. outside of the Municipality.

Lifeline Program:
Help is just a button push away; one press of your lightweight help button activates a small in-home unit. It puts you in touch with a caring lifeline monitor who talks to you to find out what kind of help is needed. They will then call one of your responders, a friend or neighbour, who will come to your home to check up on you.

The Lifeline program managed and operated out of Rock Lake Hospital.

Louise Community Support Services For Seniors
Fran Cavers, Coordinator, L.C.S.S offers a resource service to Senior's from the R.M. of Louise with office hours in Pilot Mound (825-2443) Mon, Wed, Thurs, and Friday and in Crystal City (873-2103) on Tuesday. For more information on services available please call Fran.

Meals on Wheels:
The Meals on Wheels service delivers meals to Seniors in both Pilot Mound and Crystal City. Service is available seven days a week thanks to the countless volunteers who drive and deliver the meals for the program. Volunteers from the Hospital to Crystal City citizens and from Rock Lake Personal Care Home to Pilot Mound Citizens deliver meals. It is the evening meal that is delivered.

Cost of the program is $5.00 and will be billed on a monthly basis.

Occupational Therapy:
Back to top Occupational Therapy services are available through Rehabilitation services, Boundary Trails Health Centre. Visits are made to Rock Lake Personal Care Home and Prairie View Lodge one day every two weeks.

Therapist is Jackie Derkson.

Palliative Care Program:
Palliative Care is active, compassionate care and support for the chronic and terminally ill at the time when there is no cure for the disease. The focus is on improving the person's quality of life.

Palliative care services are offered by a group of dedicated professional and volunteers who seek to provide physical, emotional and spiritual support to individuals and their families at home, in long term care facilities or in the hospital.

Consultant Pharmacist:
Rock Lake Hospital, Rock Lake Personal Care Home, and Prairie View Lodge are provided with services from the local pharmacist who provides regular visits to each facility.

Physiotherapy:
Physiotherapy services are available through Rehabilitation services, Boundary Trails Health Centre and visits are made once per week to the Rock Lake Hospital.

Physiotherapist is Gail Brewis.

Referrals/appointment are made through Laurie Treble, Health Records at Rock Lake Hospital 873- 2132 (extension 5).

Public Health:
Public Health provides families with many services including: prenatal and postnatal care, immunizations, vision and hearing screening, developmental assessments, family planning clinics, and educational presentations. Services are provided in the areas of Health Promotion, Prevention and Health Protection.

Our Public Health Office is located in the Rock Lake Personal Care Home building.

Brenda Dow, Public Health Nurse is available to the community in many different capacities. Pam provides many clinics within the community. For information or appointments please contact Pam at 825-2466.

Respite Care:
Respite care provides short-term care for those who are unable to care for themselves at home and is also intended to provide respite to caregivers. There is a bed available at Rock Lake Hospital that will provide Respite Care. Care can be arranged in advance for the specific amount of time required. Seniors Day Out Program:
Back to top Seniors Day out provides an opportunity for seniors to spend the day in the company of other seniors. Offered three days a week (Tuesday, Wednesday, Friday). The program is situated in the Seniors Day out a room provided at Prairie View Lodge. Cost per day is $7.15 per day with meals and transportation included.
